Historical facts in bullet form

Please submit all facts in bullet form so that I can compile them. I know we all have our own rip off stories, but what I'm looking for is a summary of events surrounding this company- how long operations were going on, health implications from inferior materials, different agencies that you have already notified (so we know which departments have been already addressed), the different violations, etc. I'm finding the difficulties of coordinated communication among the different agencies, local to federal, and find that an alternative medium may be the best solution. However, I will need a summary to assist in bringing more attention to this issue, so that people like me, won't have to join forums like this. Respectfully Yours.
